Please complete and sign the application form below and return by fax or e-mail. Once we receive your paid order we will instruct you where to courier your 1" X 1" colour photo and signature. DO NOT send to our address.

Payment can be made by Bank wire, Bitcoin, MoneyBookers/Skrill, Western Union and Money Gram. For FASTEST service or use Bitcoin Bank to bank wires also accepted for this product. Sorry credit cards NOT accepted for this product!

NOTE: Quality can not be rushed. All DL's are prepared from "without" the United States. So please allow up to 28 working days from the time we receive your funds, application, signature and pictures to receive your completed DL's.

Instructions for Filling out this Application

1. GIVEN NAMES. This is mistakenly known as your First and Middle name.
2. FAMILY NAME. This is commonly known as your name or surname.
3. POSTAL LOCATION: A real address will be provided. However this can NOT be used for the sending and or receiving of any personal mail.
4. COUNTRY OF ISSUE. There are only two available, but please DO NOT ask until we have received your paid order. We do this in order to protect the integrity of the product. We do not have translators, nor can we print in Asian or Aramaic fonts/symbols. NOTE. We will not provide DL's for the federal "United States" its territories or possessions.
5. Please provide a foreign address for the DL, or just let us know if you want us to provide one for you. If you leave the address part blank, we will INSERT AN ADDRESS FOR YOU! This will be a valid postal address in the country of your order.

Important Note:
Though these products are legit and not against (current) laws in most countries, non the less the authorities are NOT happy about the sale to American residents of drivers licenses and or any other ID type products. We can not and will not be held responsible in the event customs opens your order and confiscates the product. Therefore we no longer ship ID products via post. ALL ID type orders are shipped via relayed courier service only.

All ID and DL orders are at buyers risk and it is further understood and agreed that they are sold as "Novelty Items Only." There are no warranties as to what you can or can not do with these DL type products. If in doubt consult a solicitor (lawyer) in you're country of residence.
